Ed.D. Advising Forms
Being a successful doctoral student at the University of Southern California requires knowing and following many academic policies and procedures. Accomplishing certain tasks requires the use of necessary forms and contact with multiple campus offices. The following forms are most commonly used during the pursuit of your Doctor of Education degree. Please contact your Program Advisor about which form you may require or for guidance on completing any of the forms. Here are the forms most commonly used by Ed.D. - Qualifying Exam Application.doc
Identifies who will be on the Qualifying committee and serves as a prompt for the Ed.D. Program Office to perform a degree check - Outside Member Approval Form
If the third member of your dissertation committee is not a USC faculty member, please call the Ed.D. Program Office at (213) 740-9323 to see if the person has already been approved to serve as a third member. If not, you will need to secure approval for your outside member using this form. - Qualifying Exam Report
- Change of Ed.D. Advisement Committee
Once your committee has been selected this notes any changes in membership - Tier II Permission to Enroll for EDUC 595a/b
- Permit to Enroll EDPA 618 Seminar in Superintendency
- EDUC 790 Contract
Used to establish your directed readings/research course - Permission to Enroll in 790
- Permission to Enroll in EDUC 792 Critique of Research
- Permission to Enroll in 791
Used to establish and confirm your eligibility to enroll in the dissertation proposal phase - Permission to Enroll in 794b-d
Used to establish and confirm your eligibility to enroll in the dissertation phase - Permission to Enroll in 794z
- Memo of Internal Substitution
List the courses that are being substituted for catalog requirements, and attain appropriate signatures - Leave of Absence Request
Required for any semester in which you will not be registered, to ensure that continuous enrollment is not broken - Reinstatement Request
If continuous enrollment has been broken, this form is used as a request to re-enter the program - Academic Appeals Policy
